Because surgical procedures are not available within Wales, the service arranges for patients to receive operations in England. According to the health board's formal update, the decision to pause referrals followed a Quality, Safety and Outcomes Committee meeting held on 24 August 2026, where the service was escalated to Level 3 status within the commissioning oversight framework.
What services remain available?
While the surgical pathway has been halted, the health board confirmed that other core elements of the Welsh Gender Service continue to operate without interruption. Patients can still access hormone therapy support, endocrine review appointments, psychological support and speech and language therapy services. The health board also stated that individuals with existing scheduled appointments should continue to attend, where they will receive discussions about next steps and ongoing psychological support.
What triggered the review?
The independent clinical review was not entirely unexpected. A commissioning review of the Welsh Gender Service had already been planned as of January 2026, with the intention to bring in independent clinical expertise from England and allow approximately three months for the work once resources became available. By March 2026, the review was scheduled to begin by the end of the 2025-26 financial year and was expected to consider findings from the Levy Review of English gender clinics.
The immediate trigger for the pause, however, came from the referral data analysis. The health board acknowledged in its statement that the announcement "may be concerning for patients, their families, staff and the wider community" and emphasised its commitment to maintaining appropriate oversight and scrutiny of specialist services.
