Draper, 24, was visibly upset during a changeover in the third set. His serve speed had significantly dropped, and he appeared concerned about his left arm as the match slipped away.
He had already pulled out of the DC Open in Washington last week, as well as missing Wimbledon, after the injury setback.
There are now doubts over his involvement at the Cincinnati Open, where he has a wildcard entry for next week, and questions about his fitness for the US Open.
