Fatal Accident Inquiry Announced

A fatal accident inquiry (FAI) will be conducted following the death of Sarah Aitken, 59, who was found deceased on a Glasgow stairwell weeks after her admission to a mental health ward.

Sarah Aitken passed away on 15 August 2022, having been admitted to the Rutherford Ward at Gartnavel Royal Hospital on 21 June 2022.

Scope of the Inquiry

The inquiry aims to investigate the circumstances surrounding her death. This includes examining the adequacy of risk assessment, care planning, and whether NHS Greater Glasgow and Clyde adhered to relevant policies. Additionally, the inquiry will consider if any opportunities to prevent her death were missed.

The Crown Office and Procurator Fiscal Service (COPFS), Scotland's authority responsible for death investigations, stated that the circumstances of the death caused "serious public concern."

FAIs are inquisitorial proceedings designed to establish facts rather than assign blame.

Next Steps

A preliminary hearing is scheduled for 22 July at Glasgow Sheriff Court.
